Visit our website
A Hospital Direct Order Form is required to be filled out by the Prescriber for equipment to be ordered.
If the Patient requires equipment from Crescent Healthcare (contracted supplier), approval is not
required for contracted items as per our brochure up to $500.00.
If the Patient needs something outside of the contracted items, the Order Form will need to be
submitted to the WorkSafe Agent for pre-approval then it will be submitted to the supplier.
For discharges of high cost/customised discharge equipment e.g. hospital beds, mattresses, hoists,
customised commodes, etc. a quote will be required and will need to be submitted to
the WorkSafe Agent for approval.
For public hospital discharges, WorkSafe will take over funding post 30 days
for standard equipment, if it is a highly customised product a quote will be
required and needs to be submitted to the WorkSafe Agent for approval.
WorkSafe also has a PARO (Pre-Approved Repair Order) system in place
for highly customised equipment e.g. beds, mattresses, commode chairs,
where we have a $1,000.00 order for any repairs throughout a year e.g.
seats, castors, which save our therapists from obtaining quotes etc. These
repairs can be processed under this order number.

and out of the bath, easier.

The large surface area gives the user confidence and the
